JaxCareConnect is a Project of the Duval Safety Net Collaborative - Agape Family Health, Community Health Outreach, Muslim American Social Services, Mission House, Sulzbacher, Volunteers in Medicine and We Care Jacksonville.
Shared Vision: Every resident of Duval County has access to high-quality, comprehensive healthcare – regardless of insurance status or ability to pay.
What We Do
We connect uninsured Duval County residents with free and charitable healthcare.
